REGISTRATION PROCEDURE

  1. Download and complete the application form
    OR complete the application form online.
  2. Full payment must accompany registration.
  3. A minimum deposit of 50% of total fees must accompany registration along with a post dated cheque for the balance on or before June 1, 2010. Applications after June 1 require full payment.
  4. Application can be mailed, faxed, phoned, or sent online to our office.
  5. You will be mailed out a confirmation.
  6. Make cheques payable to : CANADIAN TRINITY HOCKEY SCHOOL
  7. Return completed form with proper payment to:

DISCOUNTS

(One discount per player only)

Early Bird Prizes: Will be drawn Feb. 15, & Mar. 31).
Each draw will be a registration refund! Register early to guarantee spot.

Early Bird Discount: Register before Mar. 31, 2010 and receive $15.00 off.

Family Discount: 2 or more siblings receive $15.00 off each

Group / Team Discount: Contact office for discount.

CANCELLATION POLICY

If you cancel:

All credits and refunds are subject to a $35.00 processing fee. Note: No credits will be given to any players who leave the camp once the week has started, except with a medical note.

SCHEDULE PICKUP & FREE SKATE

Tri Leisure Centre • Sun, Aug. 1 & 22 • 6:00 - 8:00 pm

Millennium Place Arena • Sun, Aug. 8 • 6:00 - 8:00 pm

Enrolled players pick up their schedule on the Sunday of your week of registration. Late registration will also be accepted if space is available. There will be a free one hour skate for players and family each Sunday during registration.
